Consent Form

 The consent form is a state mandated document governed by the

Departmentof Professional and Occupational Regulation, or DPOR.

It must be filled in every time a client is tattooed or pierced, and kept on file for 3 years.

The client must also provide a legitimate form of  identification that is copied onto

the consent form at the time of the procedure.
